We are seeking a Certified Peer Specialist-Case Manager to join our Permanent Supportive Housing Team!

The Permanent Supportive Housing Program is a cost-effective way to help people with disabilities live more stable, productive lives. Supportive housing is widely believed to work well for those who face the most complex challenges. For those individuals that are homeless, have very low to no income, and/or have serious, persistent issues that may include substance abuse, mental illness, HIV/AIDS, or other serious challenges to a successful life, permanent supportive housing is a key to success.

Under the supervision of the Site Supervisor, the Certified Peer Specialist-Case Manager will:

Duties and Responsibilities:

  • Collaborate with each client to develop and strengthen their Self-Sufficiency Action Plan according to individual goals and objectives.
  • Provide clients training in the areas of personal hygiene, accessing community resources, medical adherence, household management, or other needs as outlined in the client's Self-Sufficiency Action Plan.
  • Ensures that client interactions are person-centered, goal-focused, and reflect each client's Self-Sufficiency Action Plan.
  • Perform home visits and off-site case management meetings.
  • Provide referrals service plan for comprehensive services to ensure housing sustainability.
  • Assists residents with the transition from homelessness by coordinating with emergency shelter and outreach services.
  • Maintain a caseload.
  • Maintain contact with all housing clients on a scheduled basis that is appropriate to their particular status and needs.
  • Identifies and provides intervention for potential crisis situations.
  • Assist with securing supportive services, including mental health, primary healthcare, and substance abuse support.
  • Facilitate relationship-building between landlords, caretakers, and clients.
  • Provides transportation to appropriate community-based resources.
  • Communicates and collaborates with outside agencies and referral resources, including food stamps, Social Security, and disability benefits.
  • Enter participant information into the database in a timely matter and update as needed.
  • Completes client entries, exits, and self-sufficiency matrices.
  • Maintain complete and organized files.
  • Maintains and provides documentation on program goals and outcomes.
  • Maintains appropriate case documentation and client confidentiality.
  • Prepares and submits reports to Supervisor or Team Lead as required.
  • Other assigned duties.

Requirements

Additional Requirements:

  • Fully COVID Vaccinated
  • Must possess a valid Georgia Driver's License.
  • Must have the flexibility to be on a "call" schedule, if applicable.
  • The applicant has received or is currently receiving mental health services and has been in recovery for at least 12 months.
  • Previous experience working in a behavioral health care setting is preferred.

Minimum Training & Experience:

  • Candidates must have a primary diagnosis of mental illness or a dual diagnosis of mental illness and addictive disease, and a strong desire to identify themselves as a person with mental illness (current or former consumer of mental health services).
  • Applicants must hold a GED or High School diploma. In addition, applicants must demonstrate strong reading comprehension and written communication skills.
  • You must have demonstrated experience with leadership, advocacy, or governance, and be well-grounded in your recovery (one year between diagnosis and application to the training).
  • Applicant will have attended Georgia Department of Behavioral Health and Developmental Disabilities Certified Peer Specialist training and pass the Certified Peer Specialist certification process.
