Nurse Assessor

Within the scope of service, Nurse Assessors will meet with clients in New Hampshire to conduct medical eligibility assessments and/or nursing facility evaluations.

What youll do:

  • Review clinical documents and conduct medical eligibility assessments for persons seeking care within a community setting.
  • Review clinical documents and conduct preadmission screenings for persons seeking care within a nursing facility setting.
  • Determine the services and community-based resources necessary to respond to health needs.
  • Ensure day-to-day processes are conducted in accordance with program and company policies.
  • Maintain professional relationships with provider community and external customers and identify opportunities for improvement.
  • Responsible for attending trainings and scheduled meetings and for maintenance and use of current/updated information for clinical reviews.

What You'll Need:

Qualifications

  • Active unrestricted Registered Nurse (RN) License, within the state of New Hampshire
  • Associates Degree or Bachelors Degree Program

Knowledge, Skills, Abilities

  • Must possess a of sense of responsibility and accountability, self- motivation, and attention to detail.
  • Strong medical and clinical assessment and critical thinking skills.
  • Excellent verbal and written communication skills.
  • Ability to use independent judgment and discretion to address, resolve, and process problems impeding the service needs.
  • Proactively identify and resolve delays and obstacles.
  • Ability to work in a Team of professionals towards a common goal of completing accurate, timely, and quality-driven assessments.
  • Proficient in MS Office, Internet/Web Navigation.

Experience

  • Minimum of 3 years experience in working with or assessing individuals with complex medical and/or behavioral health needs.
  • Experience with nursing home and/or older persons population is preferred.
