Aged Disabled Waiver RN Clinical Assessor

Who we need:

The Clinical Assessor is an RN will work within the West Virginia Aged and Disabled Waiver (ADW) Pre-Admission Screen (PAS) programs. The ADW RN performs education and initial/re-evaluation assessments of Aged and Disabled Waiver participants for the purpose of determining program medical eligibility.

Primary Catchment Area: Berkeley Jefferson Morgan and Hampshire counties in WV. Ideal cities include Martinsburg, Charles Town, and Berkeley Springs, WV. Candidates will perform phone and field assessments primarily in Jefferson, Berkeley, Morgan, and Hampshire counties but will also float into surrounding counties as needed.

What youll do:

1) Travel to, conduct, and document ADW assessments within prescribed timelines (primarily field assessments in counties of coverage)

2) Schedule ADW assessments within prescribed timelines

3) Participate in inter-rater reliability activities

4) Participate in denial hearings as necessary

5) Contact physicians and facilities to confirm medical information

6) Other duties as assigned.

What youll need:

Required Qualifications

Must possess a Nursing degree (Bachelors preferred) and a valid WV Registered Nurse license.

Knowledge, Skills, Abilities

  • Must possess knowledge of the WV Aged and Disabled Waiver policies and procedures and be willing to travel to homes and facilities.
  • Candidate should be organized and have excellent customer service skills, good interpersonal communication and be flexible.
  • Applicant must be able to accurately assess medical conditions and symptoms and functional abilities.
  • Applicant must be able to work independently based remotely from their home office and have basic computer skills.

Experience

  • Must have a minimum of 2 years experience with the aging population or with persons with disabilities (preferably working within the WV Aged and Disabled Waiver program and Aged Disabled Waiver CareConnection).
