Under the direction of the Director of Central Operations, the Clinical Services Coordinator performs a variety of tasks in support of and related to patient care. This includes administrative and clinical program duties including but not limited to vaccine program coordination, CLIA/lab coordination, ongoing training, quality improvement initiatives, project management, and ensuring adherence to compliance requirements. Projects may include aspects of adult, pediatric, or womens health specialties. The Clinical Services Coordinator may also be called upon to perform direct patient care and clinic work within the scope of his or her licensure as needed.

This is a full-time position working 40 hours per week, typically Monday through Friday with periodic Saturday hours.

Tiburcio Vasquez Health Center is a non-profit community health center that is dedicated to promoting the health and well-being of our community by providing accessible, high-quality care by integrating primary care, dental care, WIC support, mental health counseling, community health education, and more.

Compensation: $22.28 - $24.00 per hour, depending on experience.

Responsibilities:

  • Assists clinic leadership on matters and participates in the development of policies and protocols for patient care.
  • Assists in the performance appraisals of personnel through documentation and input to the Director of Central Operations/ Site Operations Managers.
  • Completes regular auditing of clinic processes and documentation to ensure compliance with regulatory regulations for all clinic sites performing on-site CLIA lab work. Submits reporting of findings to Director for follow-up with Clinic Practice Management.
  • Monitors quality control, quality assurance, safety, and inspection control practices to assure compliance with internal and external regulations. Performs additional duties as assigned related to CLIA, IZ, OSHA, and other clinical compliance.
  • Administers clinical quality improvement projects. Tracks abnormal diagnostic test results, coordinate care plans with providers, and provides patient navigation services. Follows up with patients to ensure the plan of care was completed and offers assistance when needed.
  • Clinical quality projects may pertain to multiple specialties, including general adult, pediatric, or womens health. Aids with gathering metrics on results of quality improvement efforts.
  • Processing of and any resulting follow-up required for laboratory services and procedures completed by outside services not electronically interfaced with EHR.
  • Practice Management of CareSignal remote monitoring program. Duties include but not limited to: Surveillance of CareSignal member dashboard, patient outreach in response to self-reported health measurements, coordination of care with provider care team, prescription refill management, delivery coordination of DME supplies (e.g., BP monitoring cuffs), patient education, performance of patient health status checks, and chart updates.
  • Care coordination with provider care team for refill requests not electronically interfaced with EHR.
  • Processing of and any resulting follow-up required for non-formulary medications prescribed by Healthcare Providers.
  • Provides basic clinical services as needed including, but not limited to administration of medications and injections to patients as directed by the provider; immunizations; referrals to other providers and other health resources; and health instruction to patients of following examinations. Provides clinical support for RN/LVN/MA staff as necessary.
  • Assists provider upon requests during examination of patient and according to generally accepted medical standards. Explains to patient the procedure to be performed reassures and comforts patients who are apprehensive about pending examination and treatment.
  • Performs all duties and services in full compliance with TVHCs Service Excellence Standards.

Requirements

  • High school diploma (or GED), or trade certificate required.
  • Current, valid California Certification of Medical Assistant.
  • Current, valid BLS certification.

Qualifications:

  • At least 1-year experience in a clinical setting.
  • Bilingual English/Spanish preferred.
  • Proficient with project management skills.
  • Able to break complex long-term projects into specific executable tasks, track progress, assess results, and iterate as necessary.
  • Proficient with the use of electronic health records and able to assist others in use.
  • Able to relate appropriately to, communicate effectively with, and develop trusting relationships with diverse patient population and with staff.
  • Able to motivate staff; promote team building; assume leadership role within clinical area.
  • Demonstrates flexibility in regard to job duties and assignments.
  • Have the ability to accept and travel to various assignments as directed.
