315 health is looking for a staff member to work on site with one of our partners. You will support 315 healths mission in the practice office and aid in enrolling and scheduling patients in our Remote Patient Monitoring and Chronic Care Management Programs. This will include assigning medical devices and equipment to patients, as well as instructing and educating them on how to use those devices, and answering questions they may have about the program. Additionally, you will schedule the patients first interaction with their very own dedicated registered nurse. From time to time, you will also aid the medical practice and support their day-to-day operations by performing various administrative tasks. You should be able to multitask and prioritize, as well as have excellent communication skills and a professional attitude.

Responsibilities

  • Enroll patients in 315 healths Continuous Care Management Program
  • Assign equipment to patients and ensure readings are properly transmitting
  • Educate patients on devices and get patients setup and comfortable with their assigned devices
  • Schedule patients first appointment with their 315 health nurse
  • Answer patients questions about the Continuous Care Programs
  • Understand and maintain HIPAA regulations
  • Perform other duties as requested by medical office staff (may include communicating lab or imaging results, coordinating prescription refills, etc)
  • Improve the quality of care by working closely with the practice

Requirements

  • Experience in healthcare strongly preferred (MA or LPN)
  • Familiarity with various online software
  • Experience with EMRs
  • Scheduling and front office experience in a medical setting
  • Must be organized, efficient, and pay attention to detail
  • Familiarity with medical devices including blood pressure monitors, scales, glucometers, and pulse oximeters
  • Must be comfortable interacting with and educating patients, as well as answering patients questions
  • Excellent communication and customer service skills
  • Comfortable and experienced with technology
