Director of Life Sharing Location: US-MN-Saint Paul Email this job to a friend

The Director of Life Sharing will be responsible for planning, developing, grow, implementing and evaluate our new Life Share Program. In the Life Share Program, you will expand opportunities for people with disabilities to explore community integration, self-advocacy, goal setting, and discovery of personal goals.
- Listen-Learn-Lead: Model the mission and values of Accord, helping people to live their greatest lives.
- Apply the Entrepreneurial Operating (EOS) system and tools to align program initiatives with business objectives.
- Implement Work Plan activities to meet all contractual requirements of the Discovering Choices Grant, and create a sustainable business model for Life Sharing services.
- Develop and implement the Life Sharing program at Accord including all relevant policies, procedures, and protocols.
- Complete and maintain certification in Personal Outcome Measures (POM) through the Council on Quality and Leadership (CQL).
- Conduct POM interviews with individuals to help them explore community integration, self-advocacy, goal setting, and discovery of personal goals.
- Support individuals to select and contract with a Life Sharing agency as they choose their next home and family.
- Develop and oversee the program budget, including identifying potential funding sources and managing expenses.
- Monitor and evaluate the effectiveness of the program, making adjustments as needed to ensure that it is meeting the needs of the individuals served by Accord.
- Work with the marketing staff and develop Life Sharing materials that can be shared with individuals and potential families.
- Present at conferences and marketing events to promote Life Sharing services.
- Develop and maintain relationships with partnering organizations and stakeholders to support the Life Sharing program.
- Serve as a member of Accords Quality Work Team.
- Work with the Electronic Health Records (EHR) Administrator to set up and maintain program records.
- Ensure compliance with 245D licensing and other federal and state laws, regulations and administrative rules that apply to program areas.
Requirements Education: - Bachelor's degree in a related field, such as social work, psychology, or human services. Master's degree preferred.
- If you hold a Social Work degree, we expect you to be licensed by the Minnesota Board of Social Work, as required.
Skills & Experience: - Position requires a minimum of 4 years of work experience in the field of human services, preferably working with older adults and people living with disabilities.
- Minimum of 2 years of experience in a supervisory capacity.
- Knowledge of 245D programs is strongly preferred.
- Experience in program development and implementation, preferably with a focus on services for individuals with disabilities.
- Experience in budget development and management.
- Self-starter with ability to work independently and as part of a team.
- Strong organizational and project management skills.
- Proficient in GSuite, Excel, and third party analytics.
