Access Services is hiring a full time Foster Care Coordinator to provide support children and adolescents in foster care provider homes of Access Services across the Lehigh Valley, with an office location in Bethlehem, PA.

As a Foster Care Coordinator, you have a passion to provide excellent care for children and adolescents in foster care provider homes, and promote healthy bonding between each child and his/her foster family through regular visits, training and crisis intervention. You will work with the child's treatment team to develop and implement individualized service plans that address each child's social, emotional, educational, and physical needs while continuing to promote community belonging. You will coordinate and oversee the implementation of all services outlined in the service plan for each child. As a Foster Care Coordinator, you will help ensure each child's safety and well being through regular monitoring and implementation of all agency, state and county regulations.

Work Schedule:

  • General office hours: Monday-Friday: 8:30am-4:30pm
  • Flexible schedule will be set to meet with children and provider families around their availability
  • On call rotations of one week required for all coordinators


Essential Responsibilities of Foster Care Coordinator:

  • Develop, implement, update, and review individual service plans that are strengths based and person centered in collaboration with the children being served and the team.
  • Coordinate the implementation of all supports in each child's plan.
  • Participate in on call rotation.
  • Stay current in required professional development trainings, and participate in departmental meetings, trainings, and activities.
  • Promote, support, and encourage current best practices in human services field in accordance with Access Services philosophy of care and training.
  • Support and monitor provider homes to help ensure all county and state regulations are met.
  • Coordinate and assure that adequate respite care services are provided.
  • Attend all necessary court proceedings pertaining to the assigned caseload.
  • Supervise all visits with children and family as required by court/county.
  • Provide all necessary transportation for child.



Requirements

Education:

  • Bachelor's degree required, preferably in human services, social work, psychology, education, or criminal justice.

Experience:

  • Two years of direct experience with children and/or adolescents, case management, or counseling experience is preferred.

Knowledge, Skills, and Abilities:

  • Excellent organizational abilities and oral/written communication
  • Effectively interact with all levels of staff, and as a supportive team member
  • Proficiently navigate multiple priorities and utilize Microsoft Suite

Pay: Starting at $18.00/hourly, with increases based on education and experience.
