A Case Manager provides direct services to Transitional Age Youth (TAY) and Adult Clients and functions as a member of an assigned FSP Team. In addition, Case Managers are responsible for the behavioral case management of the clients being served, as well as, responsibility for providing specific linkage services. The position involves working designated shifts under the direct supervision of the FSP Clinical Supervisor or FSP Director.
Requirements
1. Bachelor's degree related to the fields of psychology, social work or other majors defined by the Department of Mental Health as a related degree OR AA degree in Human Services, Social Work or Psychology, plus two years experience working with high risk youth.
2. Valid California driver's license, good driving record, current vehicle insurance and must be able to drive to fulfill the duties of the position.
3. Bilingual Spanish, preferred.
4. Must have internet, a computer with a camera, and Microsoft Office or a compatible program.
5. COVID-19 Vaccination and booster (when eligible) is required for this position. Accommodations will be considered with proof of a legal exemption.
Competitive Rate:
- Tier 1: $21.63- $26.92 per hour.
- Tier 2: $26.93- $31.25 per hour.
