Summary of Job Functions:



Accepting Cases from Illinois Department of Aging (CCU) and Illinois Department of Human Services, affiliated programs with Prudential and Private Sources

Preparing service calendars

Assisting clients with questions

Monitor field staff compliance with mandatory in-service training

Provide emotion support and encouragement to clients and homemakers

Supervise field staff compliance with timesheets, Electronic Visit Verification (EVV) and attendance

Acting as a liaison between client homemaker/CNA and CCU

Maintain client personal files by documenting client contact and activities related to client services

Provide daily activity reports to the Administrator

Update client and field staff schedules

Resolve family-client conflicts

Maintain POC Compliance percentage of >90%

Monitor EVV date in the Care When system on the daily basis

Document all communication in the Care when system and homecare software pertaining to homemaker's schedules

In-home supervisory visits.

Position Qualifications:



A High School Diploma or General Education Diploma

Demonstrated Management Skills, Combination of skills and experience that indicate a person has an ability to perform the supervisory activities

Excellent Organizational and Communication Skills

Certification of Completion of Illinois Department on Aging sponsored Community Care Program training

Spanish Speaking is a plus.
