Ultimate Care NY, a leading provider of home health care services for the past 20 years, is seeking a professional and compassionate Home Health Aide fluent in Cantonese for our Downtown Brooklyn location. Responsibilities

• Assist patients with daily living activities, such as hygiene, grooming, and toileting
• Assist with mobility, transfers, and positioning to ensure patient safety
• Provide emotional support and companionship to patients
• Administer medication reminders and assist with medication management
• Record and report patient progress and any changes in their condition to the nurse supervisor
• Participate in care plan development and maintain communication with clinical staff
• Ensure compliance with all healthcare regulations and safety standards

Requirements

• Fluent in Cantonese (Speaking, Listening, Reading)
• Valid and current HHA Certification
• Minimum of 6 months of experience in home health care or a related field
• High school diploma or equivalent
• Ability to work independently and as part of a team
• Strong communication and interpersonal skills
• Ability to read and understand care plans, implement best practices, and document reports accurately
