Phoenix Home Care and Hospice is looking for a Home Health Care Coordinator to join our Jefferson City, Mo Team!
This position is responsible for staffing and coordinating the day-to-day care delivered by field staff members who are working in the homes of clients. This position offers the right candidate a challenging position with an opportunity for growth. Candidate must be highly organized, detail-oriented, able to multi-task, & think on their feet in a fast paced, demanding, and rewarding position.

Job Duties:
- Responsible for scheduling Nurses to provide Home health services.
- Maintains clear, concise documentation.
- Must be willing to do occasional field staffing
- Responsible for some initial face to face visits.
- Performs other duties deemed necessary.
- Fast-paced and occasionally stressful work environment with tight deadlines.
Requirements:
- Ability to work in a professional manner.
- Must work well both independently and in a group setting.
- Prior office experience.
- Prior Scheduling and Coordinating Experience preferred.
- Prior Home Health experience a plus!
- Professional appearance.
- Proficiency with Excel and Word.
- Excellent attendance and punctuality.
- Problem solving skills and ability to multi-task.
- Excellent Communication Skills.
- Positive, Professional attitude.
