As our Hospice Chaplain, you will play a critical role in providing essential support and guidance to our patients and their families in their time of need.
Responsibilities
- Providing spiritual and emotional support to hospice patients and their families
- Assessing the spiritual and emotional needs of patients and creating individualized care plans
- Collaborating with interdisciplinary teams to provide a comprehensive approach to care
- Offering bereavement support to family members after the passing of a loved one
- Documenting patient visits and care provided in accordance with company policies and procedures
- Participating in interdisciplinary team meetings and providing input for patient care
- Supporting the company's mission and values by maintaining a positive and professional demeanor
Requirements
- Master's degree in Divinity or Theology
- Ordained or endorsed by a recognized faith group or equivalent experience
- Minimum of one-year hospice chaplain experience preferred
- Ability to work effectively with patients and families with diverse beliefs and backgrounds
- Excellent communication and interpersonal skills
- Ability to maintain confidentiality and work in a highly sensitive environment
- Strong organizational and time management skills.
