ORGANIZATION OVERVIEW
Founded in 2004, Brilliant Corners provides innovative housing and housing-related services to those transitioning from or at risk of homelessness or institutionalization. We develop, own and manage multi-family supportive housing and licensed residential care homes. Additionally, we implement a broad array of scattered-site, one-on-one supportive housing, and clinical case management programs through partnerships with developmental services, homeless services, veterans services and health care sectors.

PROGAM SUMMARY
The Housing and Disability Advocacy Program (HDAP) at Brilliant Corners serves people experiencing homelessness in Riverside County who are likely eligible for disability benefits and is supported by the Riverside County Department of Public Social Services (DPSS). HDAP provides benefits advocacy, intensive case management, and housing assistance. The goal is to transition individuals out of homelessness and into permanent housing, in order to obtain stability in all areas of their lives and thrive within their communities.
POSITION SUMMARY
The Case Manager will provide intensive case management services and disability benefits advocacy to a caseload of approximately 20 individuals experiencing homelessness in Riverside County who are disabled or likely disabled. The Case Manager plays a critical role in supporting clients to secure disability benefits and thrive in their new homes through a client-centered approach to services.
POSITION RESPONSIBILITIES
1. Accept and process referrals from Riverside County DPSS and other DPSS partners.
2. Conduct outreach and engagement, including in interim housing and encampment settings, to potentially eligible HDAP participants by utilizing established partnerships with local homeless outreach teams.
3. Complete assessments and re-assessments, including a comprehensive psychosocial assessment, in order evaluate clients needs and strengths.
4. Develop and implement individualized case management services plans that ensure a secure source of income, housing stability and identify short and long-term goals.
5. Assist clients throughout the entire application process for SSI/SSDI benefits, including obtaining the relevant documents and supporting the appeals process if necessary.
6. Provide support throughout the housing process in collaboration with the internal Brilliant Corners housing team, including but not limited to temporary housing placement, unit identification, application submissions, lease signing, and ongoing housing retention.
7. Provide education to clients on tenant rights and responsibilities as well as eviction prevention counseling and advocacy.
8. Provide linkage to community resources, benefits, and services which may include physical and mental health, substance use treatment, education/vocational programs, health insurance, disability benefits, and other sources of financial assistance.
9. Meet with clients frequently and regularly, including in the home once housed, and accompany clients to appointments as needed.
10. Provide crisis intervention and ensure that clients have access to 24/7 crisis lines.
11. Provide life skills education and assistance, including budgeting, apartment upkeep, stress management, conflict resolution, etc.
12. Facilitate transportation assistance and coordination.
13. Coordinate with clients care providers, including medical providers, in-home caregivers, and occupational therapists to ensure clients needs are being met.
14. Develop transition plans with clients that include connection to longer-term housing and appropriate ongoing support.
15. Participate in case conferences to review clients progress and address any challenges.
16. Build and maintain partnerships with community organizations, service providers, public entities, and landlords to ensure that clients have access to comprehensive services.
17. Maintain updated and accurate client files and documentation.
18. Participate in staff meetings and other agency events as requested.

PROFESSIONAL EXPERIENCE
- At least 1 year of experience providing intensive case management services and a Bachelors Degree in Social Work, Psychology, Sociology or a related field OR at least 2 years of experience providing intensive case management services
- Experience working with individuals with chronic medical and/or behavioral health conditions, ideally with individuals experiencing homelessness.
- Experience with disability benefits advocacy and the SOAR model is preferred.
KNOWLEDGE, SKILLS, AND ABILITY
- Strong interpersonal and client engagement skills
- Excellent verbal and written communication skills
- Flexibility regarding scheduling and prioritizing of tasks
- Willingness to travel and spend significant time in the field
- Basic computer knowledge (MS Word and Excel required)
- Familiarity with evidence-based interventions including Harm Reduction, Trauma Informed Care, and Motivational Interviewing
- Ability to work independently and utilize critical thinking skills in decision-making situations
- Good organizational and record-keeping skills
- Possession of a valid, clean California drivers license and a personal insured vehicle are required
- Bilingual in English/Spanish preferred
