The In Home Care Coordinator assists persons with physical, mental or multiple disabilities in accessing home care services. The In Home Care Coordinator provides case management services, information, referral services and performs record-keeping functions.

Job Duties:

  • Assists in evaluation of consumers for program eligibility.
  • Responsible for documenting services provided according to policy and procedures.
  • Maintains clear, concise and accurate records in the CDS program.
  • Responsible for keeping accurate records of community contacts.
  • Maintain records of completed requests for information and/or Referral.
  • Makes frequent home visits to consumers in several counties.
  • Perform 8-10 company-wide audits per month.
  • Perform monthly audit reports for all cities.
  • Maintain spot-check case management notes and activity schedules.
  • Develop training forms
  • Marketing of facilities and create/provide calendar of events.
  • Performs other duties deemed necessary by In-Home Manager/Executive Director.
  • Provide/maintain 4 core services assessment tools per month.

Requirements:

  • Must be a minimum of 18 years of age
  • High school diploma or equivalent
  • Supervisory Experience preferred
  • Office Setting Experience
  • One Year minimum experience working with elderly and/or disabled
  • Ability to work independently with excellent organizational skills
  • Ability to communicate effectively and courteously with consumers and perform in-depth interviews with consumers.
  • Must work well in a fast paced environment and adapt quickly to changing conditions.
