Lead RN MDS Coordinator -No on-call! 



We are seeking an experienced and dedicated RN MDS Coordinator for a lead position with a top-rated Skilled Nursing facility in the Western New Haven County area of CT. 



The Lead MDS Coordinator coordinates all components of the MDS process, which includes the MDS, Care Area Assessment process, and care plan development and review within federal and state regulations.  This position will be the lead coordinator for CT Case Mix reimbursement system. This position works with all disciplines to ensure regulatory compliance and positive clinical outcomes while maximizing reimbursement.



Responsibilities include but are not limited to:

*Scheduling and completion of MDSs per OBRA and Case-mix guidelines

*Scheduling UDAs for interdisciplinary team members

*Managing case-mix documentation and analyzing facility data

*Coordination and attendance of Long Term Care Plan Meetings with residents and their families

*Attendance at morning clinical rounds and other interdisciplinary meetings that support risk review

*Works with IDT collaboratively to develop person centered care plans

*ICD-10 coding and sequencing

*Assist with Activities of Daily Living training and other key documentation areas for new hires and through ongoing continuing education to existing staff.

*Provide back up support to PPS/MDS Coordinator and Case Manager as needed



Must be able to plan, organize, prioritize and work independently to meet deadlines. Experience with Point Click Care, Rehab Optima and PPS/RUG IV a plus! This position has great resources and regional support and NO On-call required.
