Touchpoints at Home an affiliate of Ultimate Care a leading home health care services provider in Connecticut is hiring a Licensed Practical Nurse (LPN) in the Greater Hartford Area. This is a great opportunity for a skilled and compassionate LPN to be part of a team that has been supporting thousands of New Yorkers for the past 20 years. As an LPN at Touchpoint, you'll be responsible for providing a high level of care that meets the tailored needs of each patient.

Responsibilities

• Provide direct patient care, including administering medication, supervising the administration of medication by others, and performing treatments as ordered by the physician.
• Observe patients' physical, mental, emotional, and social conditions and report findings to the physician.
• Prepare and maintain patient records and document all patient care activities according to agency policies.
• Communicate with patients, their families, and other health care providers to ensure a coordinated and effective care plan.
• Ensure a safe and clean environment for patients by complying with infection control and safety procedures.
• Participate in Quality Assurance activities as assigned by the Director of Nursing.
• Other duties as assigned by the Director of Nursing.

Requirements

• Current LPN license in the state of Connecticut.
• Minimum 1-year nursing experience preferred.
• Ability to work independently and as part of a team.
• Excellent communication and interpersonal skills.
• Must have a valid driver's license and access to a reliable vehicle.
• Flexibility to work day, evening, and weekend shifts.
• Ability to pass a background check and drug screening.
