Sombrillo Nursing & Rehabilitation, an integral part of Los Alamos Retirement Community, is currently seeking a qualified and caring MDS Coordinator (RN) (LPN) for our skilled nursing facility residents to develop care plans and ensure the documentation for the care provided is accurate and timely.

Duties & Responsibilities:

  • Conduct and coordinate the development and completion of the resident assessment (MDS) in accordance with current rules, regulations, and guidelines that govern the resident assessment and care plan process.
  • Maintain an effective, friendly working relationship with health professionals, physicians, consultants, and governmental agencies that may be involved in the resident assessment/care plan functions of the facility
  • Assist the resident and Discharge Planning Coordinator in completing the care plan portion of the residents discharge plan. Assist the resident and Discharge Planning Coordinator in planning the nursing services portion of the residents discharge plan.
  • Participate in facility surveys (inspections) made by authorized government agencies.
  • Monitor the facilitys Quality Measures reports to ensure that appropriate corrective action can be implemented when potential problems areas occur as directed.
  • Ensure that all assessments are completed and transmitted in a timely manner. Report problem areas to the administrator.
  • Perform administrative duties such as completing medical forms, reports, evaluations, studies, etc., as necessary.
  • Agree not to disclose assigned user ID code and password for accessing resident/facility information and promptly report suspected or known violations of such disclosure to the Administrator.
  • Agree not to disclose residents protected health information and promptly report suspected or known violations of such disclosure to the Administrator.


Requirements

Requirements & Qualifications:

  • Current, unencumbered, active license to practice as a Registered Nurse or as a Licensed Practical Nurse in NM.
  • Must be able to read, write, speak, and understand the English language.
  • Must demonstrate the knowledge and skills necessary to prove care appropriate to the age-related needs of the residents served. Must be a supportive team member, contribute to and be an example of team work and team concept.
  • Must possess the ability to make independent decisions when circumstances warrant such action.
  • Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public. Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to nursing care facilities.
  • Must possess leadership and supervisory ability and the willingness to work harmoniously with and supervise other personnel.
  • Must possess the ability to plan, organize, develop, implement, and interpret the programs, goals, objectives, policies and procedures, etc., that are necessary for providing quality care. Must have patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to handle difficult residents.
