Topanga Terrace, a leading healthcare provider in the region, is seeking an experienced MDS Coordinator to join our team. The MDS Coordinator will play a crucial role in ensuring that our patients receive the highest quality care possible. The successful candidate will be responsible for managing the Minimum Data Set (MDS) information and overseeing the accuracy and timeliness of assessments and care plans. This position requires a highly organized, detail-oriented individual with clinical knowledge and strong communication skills.

Responsibilities

• Conduct and coordinate the development and completion of the resident assessment (MDS)
• Maintain accurate and timely records of resident assessments
• Ensure compliance with all state and federal regulations related to MDS assessments
• Develop and implement care plans that are individualized to meet the needs and preferences of each resident
• Collaborate with other healthcare professionals to ensure that resident care is coordinated and comprehensive
• Assist in staff education and training related to MDS and care planning
• Participate in interdisciplinary team meetings to discuss resident care and progress

Requirements

• Current LVN or RN license in the state of CA
• Minimum of 3 years of experience in long-term care nursing, with at least 1 year of experience in MDS coordination
• Proven ability to manage multiple tasks simultaneously while maintaining attention to detail
• Strong leadership skills and the ability to work effectively within a team environment
• Knowledge of state and federal regulations related to long-term care facilities and MDS
• Excellent communication and interpersonal skills
• Proficient in Microsoft Office applications and electronic medical record systems
