Position: Matter Mobile, Primary Care Provider

Location: Knoxville, TN (multiple sites in the patient homes in the general Memphis Metropolitan Area)

Hours: Monday Friday | 8:00am 5:00pm (local time)

The Day-to-Day:

The Matter Mobile, Primary Care Provider at Matter Health is a clinical leader of patient care in the communities we are fortunate to serve. As a leader of patient clinical care, you will be an active and visible member to the community. The Provider visiting patients in their residencies, will assist residents with the coordination of all their medical needs and social services. Building sincere relationships with your patients and adjusting treatment plans specific to the patients needs. You will combine traditional clinical skills with effective community engagement, problem solving and strong organizational skills in a truly rewarding role.

Our Matter Mobile, Primary Care Provider is energized by the idea of providing exceptional in-home services to our patients through holistic, evidence-based care. You will conduct physicals, evaluate well-being and devise care plans specific for each member following a value base care model. You are a consistent resource for our members answering any questions they may have; no question is too big or small!

The Matter Mobile, Primary Care Provider will also support the Membership Consultant in growing our member base - whether engaging directly with individuals in the community, attending events or helping to organize them.

Patient-focused quality and safety

  • Provide comprehensive in-home care for a panel of patients residing in a group of properties within a designated geography
  • Evaluate primary care, acute, chronically ill, and transitional care patients
  • Provide patient counseling, medication management and education on health maintenance
  • Document all patient care within Athena, Matter Healths EHR
  • Coordinate the care of our patients with internal and external colleagues ensuring the highest standard is provided in all settings across the continuum of care
  • Phone triage, outreach, patient education, population health leadership
  • Participate in an after-hours on-call rotation

Patient Experience

  • Maintain a focus on the overall patient experience
  • Tailor communication style to effectively influence quality outcomes and building relationships
  • Maintain patient confidentiality in accordance with HIPAA and PHI

Community Building

  • Work closely with the Care Navigator and Membership Manager Consultant to create a thriving community at the property in which your clinic is located properties encompassing your patient panel
  • Work to build personal relationships and social influence with all members of the community
  • Attend social events and become ingrained in the lives and well-being of each of the patients you serve

Autonomy & Accountability

  • Work independently in your clinic to prioritize and solve problems
  • Take initiative to help build and improve Matter Mobile, improve your clinic while advocating for your patients
  • Actively participate in professional development opportunities and groups within your market
  • Maintain professionalism of individual actions and self-awareness of how they impact your panel, market, Matter health, and the healthcare industry.
  • Own the success of your panel through implementation of the clinic level business plan designed to meet identified business goals

Requirements

What Leads to Success?

  • Passion for Matter Health model, vision, and patient population
  • 3 years' experience as an Advanced Practitioner
  • 1 year of providing clinical experience in homes in the Memphis community
  • An Active DEA certification with no restrictions
  • A Masters degree and certification by one of the Nurse Practitioner or Physician Assistant certifying bodies
  • A valid, active and unencumbered state Nurse Practitioner or Physician Assistant license
  • Active unrestricted drivers license and a clean driving record
  • Reliable vehicle with current registration and state minimum insurance
  • US Work Authorization Required

Nice to Haves:

  • A specialization in the field of Gerontology with applicable experience
  • Experience with and a passion for value-based care
