AlediumHR, one of North America's leading telehealth and healthcare recruiting firms, is actively recruiting a Medical Director based in San Jose, CA, for our client's new PACE program.
As the Medical Director of their new PACE program in San Jose, CA, you will be at the forefront of a revolution in healthcare. PACE (Program of All-Inclusive Care for the Elderly) is a unique model of care that provides comprehensive medical and social services to seniors in their homes and communities.
Core Responsibilities:
- Providing primary care and acute illness evaluation and treatment for a panel of approximately 150 participants.
- Leading an interdisciplinary team of healthcare professionals to develop and implement individualized care plans.
- Collaborating with network providers and other stakeholders to ensure seamless care coordination.
- Overseeing all aspects of the PACE program, from clinical operations to financial management.
Core Experience:
- A strong clinical background in internal medicine or family practice
- Board certification in geriatrics (preferred)
- A deep understanding of the PACE model of care
- Excellent leadership and management skills
- A passion for making a difference in the lives of seniors
