Touchstone Health of San Antonio is actively seeking a qualified and organized Medical Records & HR Assistant for our growing Home Health and Hospice Agency. This individual is responsible for all areas related to medical records integrity and maintenance as well as assisting our Human Resources Director with employee file maintenance & data entry for payroll. This position requires exceptional organization, sound judgment, a flexible attitude, and strict adherence to protect patient privacy and employee information. Responsibilities:

• Maintains all medical records
• Answer Phones
• Indexes, files, and processes patient records
• Tracks Claims and notifies Director of further documentation needed
• Maintain spreadsheets
• Filing and pulling medical records
• Scanning
• Ability to verify Medicaid benefits
• Data entry of patient information
• Reviews daily activity logs to ensure each visit code has corresponding visit note
• Understands concepts, documentation. and regulations regarding Medicare/Medicaid guidelines
• Accept other assignments as appropriate
