Job purposeThe Primary Care Physician is responsible for providing care in River Family Health Center. They are responsible for clinical decision-making as it relates to care, treatment and follow up care provisions, teaches disease prevention and health wellness. They are responsible for participating and supporting Quality Improvement activities for the institution and the clinic area. Duties and responsibilitiesCLINICAL PRACTICE Competency: Provides evaluation and treatment to River Family Health Center patients following evidence-based guidelines Expectations: Provides direct patient care on an on-going basis within their scope of practice. Assesses patients Performs history & physical Determines from clinical assessment a diagnosis and coordinates care with specialists when needed. Works with other care practitioners to ensure appropriate education and discharge planning are provided. Initiates immediate patient intervention when an emergency medical condition is determined. Makes arrangements for the patient to be transferred to the Emergency Department. (May or may not accompany the patient.) Teaches disease prevention and health wellness. Adheres to policies and procedures for documenting care provided in the setting. Ensures that documentation is concise and appropriate. Collaborates with the Administrator / Physician Supervisor in developing and revising patient care standards, protocols, policies and procedures.ADMINISTRATIVE RESPONSIBILITIES:Competency: Supports hospital mission for rural health care.Expectations: Is knowledgeable of the hospital mission. Represents the hospital in a positive way with outside individuals, agencies and etc. Participates in marketing services of the satellite area. Participates in community activities, when able, to promote the clinic services. Participates with your Administrator to evaluate needs for the clinic setting on an on-going basis. This is to include both human resources and material (link removed)petency: Is self-regulated in adhering to the medical staff by-laws, rules, regulations and hospital policies and proceduresExpectations: Maintains membership on medical staff committees as assigned. Participates in investigation, analysis and corrective action plans for complaints related to the (link removed)petency: Provides oversight for Non-Physician Providers in the outpatient settingExpectations: -Evaluates care given in the setting by other practitioners. Participates in providing input for performance review of practitioners in the setting. Ensures that practitioners adhere to standard protocols, policies and procedures and outside agency regulations. Is responsible for communicating problems and/or issues to your administrator and assisting with planning corrective actions steps when necessary. Participates in the recruitment, hiring and orientation of Providers in the clinic setting. Assumes other duties as assigned within scope of practice.PROFESSIONAL RESPONSIBILITIES/DEVELOPMENT Competency: Keeps updated in Medical Practice Specialty. Expectations: Participates in continuing education programs (Topics should include areas of specialty practice) Maintains current licensure in New York State Competency: Adheres to Hospital Policies and Procedures for certifications & in-service training. Expectations: Maintains current certifications including: BLS & ACLS Completes hospital mandatory in-service program updates on an annual basis Participates in the recredentialing processQUALITY IMPROVEMENT Competency: Works closely with the Hospital Quality Improvement Director to evaluate Quality Care Standards and implement quality improvement actions Expectations: Ensures compliance with the New York State Department of Health Code 405. Participates in Quality Improvement activities. Provides input, when requested, to your Administrator/Physician Supervisor related to other practitioner's performance in the clinic setting. Participates with Director of Quality Improvement in planning, promoting, and conducting quality improvement activities for the calendar year. Participates in investigation of sentinel events and assists in recommendations for corrective action. Accepts other responsibilities and duties as assigned by the Administrator/Physician Supervisor. Competency: Participates in formulating and revising clinical policies and procedures for the Outpatient Clinic areas Expectations: Works with Physician Supervisor and other members of the healthcare team to develop and implement policies and procedures relating to clinical practice for the clinic. Assists with orienting practitioners to revised/ new policies and proceduresQualifications Graduate of an accredited AMA/AACOM approved medical school. Board Eligible or Board Certified in a Medical Specialty and/or verified designated training in Family Medicine or IM/Peds Current New York State License, Current BLS and ACLS Credentialing approval for delineation of privileges from the Medical Staff and Board of Trustees at River Hospital.

Shift: M-F, no wkends, no CALL, no holidays
