Essential Job Duties: Partner with Atria physicians to provide exceptional care in a state of the art facility, as well as providing care to our members in the comfort of their homes Works collaboratively with CMOs to provide Preventive Care, Well Visits & Comprehensive Health Assessments in the Institute, also with direct and indirect supervision for urgent Home Care visits Flexibility to work weekends and off hours for on-call coverage for home care visits and Institute coverage, as necessary Adult Care NPs & PAs must have the ability to work fluidly and competently across multiple disciplines, including Internal Medicine, Cardiology, Neurology and Geriatrics for cross coverage and to meet operational needs of the Institute to cover staff for illness, personal leave, vacation or continuing medical education Pediatric NPs & PAs must have ability to provide comprehensive Primary Care to the pediatric population Is competent to provide care to members based upon the age groups treated Adaptability to gain training and demonstrate competencies in a variety of Point of Care Testing modalities and devices. Flexibility to perform clinical, administrative and clerical tasks outside routine scope of practice Opportunity to perform Exercise Physiology and Mobility Testing with Diers/Dari Providing Home Care Services for minor procedures, such as insertion of peripheral IV for medication administration, wound care, vaccinations and phlebotomy techniques. Documentation and appropriate maintenance of Medical Records for members Telehealth visits & Electronic prescribing of medications as needed EMR data integration Opportunities for Growth & Development through involvement in various initiatives such as committees for Quality & Patient Safety, Protocol & Procedure Development and Review Processes Maintains a high quality workforce to assist in the achievement of the Organizations Mission.
