Marley Medical is seeking a skilled Nurse Practitioner, Diabetes Specialist to join our innovative medical practice. Our virtual primary care clinic is dedicated to managing chronic diseases and reestablishing primary care at the center of chronic disease management. We provide patients with the convenience and comfort of managing their chronic conditions from home. The Nurse Practitioner, Diabetes Specialist will be responsible for providing specialized care to patients with diabetes, adopting the latest treatment modalities and providing comprehensive support to patients in managing their disease. The ideal candidate will be passionate about patient care and dedicated to providing excellent healthcare services that exceed patient expectations.

Responsibilities

• • Provide clinical care and treatment for patients with a variety of cardio-metabolic diseases including diabetes, managing the various aspects of their condition
  • Help to craft clinical protocols to be used by other healthcare providers in the treatment of diabetes.
  • Educate patients on the latest treatment options, including the use of devices and technology in the management of their condition
  • Develop individualized care programs for each patient, incorporating their lifestyle, goals, and preferences
  • Monitor and track patient progress, evaluating the effectiveness of treatment plans and making necessary adjustments
  • Collaborate with other healthcare professionals to provide comprehensive care, managing the complexities of patients chronic conditions
  • Maintain accurate and thorough medical records, ensuring that patient information is up-to-date and accessible to other members of the healthcare team
  • Stay informed on the latest developments and advancements in diabetes care, continuously improving clinical knowledge and expertise

Requirements

• Current certification and licensure as a Nurse Practitioner in at least one of the following states: IL, MI, MN, MO, OH or WI )
• Certification as a Diabetes Specialist, either through the American Nurses Credentialing Center or the Certification Board for Diabetes Care and Education (CBDCE)
• A minimum of 5 years advance practice clinical experience in the management of diabetes
• 3+ years of virtual care experience.
• Strong clinical skills, with an ability to develop individualized care plans and treatment options based on patient needs
• Excellent communication skills, with the ability to convey complex medical information in a clear and understandable manner
• Strong organizational skills, with an ability to manage a patient caseload and maintain accurate medical records
• Familiarity with electronic health record systems and other health technology tools
