Position Details:
The Lead Palliative Care APRN will play a crucial role in providing compassionate care to patients with serious illnesses. This position combines clinical expertise with administrative responsibilities. The ideal candidate will have extensive experience in the field of Palliative Care and will lead a team of healthcare professionals.
Responsibilities:
Clinical (80%):
- Patient Assessment and Management:
- Conduct comprehensive assessments of patients physical, emotional, and psychosocial needs.
- Develop individualized care plans to address symptom management, pain control, and quality of life.
- Collaborate with interdisciplinary teams to ensure holistic patient care.
Direct Patient Care:
- Provide direct clinical care to patients and families, including symptom management, counseling, and end-of-life discussions.
- Coordinate with other healthcare providers to optimize patient outcomes.
Education and Support:
- Educate patients, families, and caregivers about palliative care principles.
- Offer emotional support and guidance during difficult times.
- Oversee PCIM resident rotation through Palliative Care Department
Administrative (20%):
- Team Leadership:
- Supervise and mentor a team of 3 APRNs and 2 social workers.
- Foster a collaborative and supportive work environment.
- Ensure efficient patient flow and effective communication within the team.
- Participate as a member of the Wolfson Palliative Care Board
Quality Improvement:
- Participate in quality improvement initiatives to enhance patient care.
- Monitor clinical outcomes and implement evidence-based practices.
Program Development:
- Contribute to the development and enhancement of palliative care programs.
- Collaborate with hospital leadership to improve processes and policies.
- Development of Palliative Care APRN clinical training program and guidelines
Resource Management:
- Work closely with administrative staff to manage resources efficiently.
- Oversee scheduling, budgeting, and staffing.
Qualifications:
- Masters degree in nursing (MSN).
- Active APRN license in the state.
- Minimum of 5 years of clinical experience as APRN in inpatient Palliative Care.
- Strong leadership, communication, and organizational skills.
- Ability to collaborate effectively with interdisciplinary teams.
- Experience in management, program development and quality improvement

Location: 100 Grand Street, New Britain, CT and 435 Lewis Ave, Meriden CT
Setting: Inpatient, Palliative Care
Schedule: Full-time, Monday-Friday.
Call schedule: No call
