Requirements

• Masters Degree in the field of Occupational Therapy or equivalent
• CPR Certification.
• Unencumbered Occupational Therapy (OTR/L) license for New York.
• Experience in long-term Occupational Therapy care and/or geriatric and/or pediatric care.
• Proficient in Occupational Therapy Electronic Medical Record (EMR) documentation (we utilize HelloNote).
• Exceptional patient service skills and ability to demonstrate clinical excellence.
• Bilingual in English & Spanish a plus

Responsibilities:

• Conduct home visit Occupational Therapy assessments of referred patients.
• Provide direct patient care to assess medical condition, functional capabilities, limitations, and potential for rehabilitation.
• Establish and administer treatment programs with specific goals determined by the patient's capacity and tolerance, under the direction of the Physician.
• Set realistic and achievable goals for patients utilizing authorization, document and record their condition, and educate patients and families on the plan of care.
