Our client, a healthcare organization based in Urbana, IL, is seeking a dedicated Palliative Care Nurse for their team.

The Clinic Nurse role requires a systematic approach to nursing, incorporating assessment, planning, implementation, and evaluation. The nurse will provide healthcare services within the scope of practice defined by the Illinois Board of Nursing to individuals, families, and communities.

Responsibilities:

  • Deliver competent health care to patients and families in the assigned area of practice.
  • Adhere to established standards of care and scope of practice.
  • Ensure complete and accurate documentation of care.
  • Provide health and wellness education to individuals and groups.
  • Support the development of staff and healthcare students as a preceptor.
  • Assist in initiatives aimed at improving care quality and clinical outcomes.
  • Collaborate with the director in developing departmental standing orders.
  • Coordinate patient scheduling, referrals, and intake to ensure continuity of care.
  • Provide care coordination for ambulatory Palliative Care patients.
  • Maintain communication with PCP and other team members within the organization.
  • Room patients and collect pre-visit symptom assessments for the outpatient clinic.
  • Complete necessary preauthorizations and insurance documentation.
  • Assist in tracking program quality metrics and data collection, if required.
  • Act as the point of contact for the Palliative Care team, answering calls and emails.
  • Help coordinate patient flow, consults, and other processes.
  • Delegate tasks appropriately to the office coordinator.
  • Initiate enrollment of new patients into the palliative care program and assess their needs.
  • Conduct telephonic outreach to schedule necessary appointments.
  • Round on inpatients under palliative care and document interactions in EPIC.
  • Conduct follow-up calls as per provider instructions to assess patient needs and overall condition.
  • Use standardized templates to ensure accurate documentation of patient interactions.
  • Participate in interdisciplinary team meetings and morning huddles with providers.

Requirements

  • College Diploma in Nursing.
  • Registered Nurse (RN) in Illinois
  • Basic Life Support (BLS) certification
  • Minimum of two (2) years of nursing experience.
