Planning LTC Onboarding and Training

Work with Regional Hospital Coordinators and identified subject matter experts to review the Healthcare Resource Tracking System and develop a comprehensive training strategy for each Long Term Care facility in a Region. Coordinate, conduct, and record remote meetings with Nursing Home Administrators. Review and modify training materials, plan and conduct training(link removed) The strategy should include a local plan to onboard LTC facilities based upon state level direction. Produce weekly status reports and evaluate data quality of submitted data. Review and modify facility profile including; beds and services, specialty services, resources, contacts and users. Document all requirements identified during training.
LTC Skilled Nursing Facilities have been and are still being hit hard with COVID-19. The Regional Hospital Coordinators (RHCs) have no visibility of their status and/or needs. We are proposing that LTC Skilled Nursing facilities become part of the Healthcare Resource Tracking System (HRTS), a system that has been used successfully by acute care hospitals across the state for 10 years. The goal is to have 100% of these nursing facilities in TN actively using HRTS daily. This will help us provide the information that is being asked of them by multiple agencies and relieve the burden of administrative tasks, as well as provide support throughout this Pandemic and set up TDH for long term continued support for the other disasters that will continue regardless of COVID-19.
If we do not accomplish the necessary training of this system, we may jeopardize the success of the program by losing engagement and long-term participation. This may result in a less prepared disaster response community affecting the life and property of Tennesseans.
