Summary

Responsible for processing new patient enrollment forms, ordering patient devices, tracking shipment of devices, and scheduling first contact with patients nurse.

Responsibilities

• Receiving new patient enrollment forms via the practices EMR
• Verify that patient consent form is uploaded in the EMR as well

• Input the new patients information into the monitoring platform
• Order the necessary medical equipment for the patient and monitor the tracking of that equipment
• Within 24 hours of the equipments arrival at the patients home:
  • Contact the patient and schedule an appointment with the patients nurse
  • Ensure patients medical devices are turned on and the first reading is completed

• Follow proper telephone etiquette and protocol when answering or making phone calls
• Follow-up on no-show new patients until patient has successfully communicated with their nurse
• If patients preferred language is anything other than English, coordinate appropriate communication to meet the needs of the patient
