The Physical Therapist at MiHom will be responsible for providing comprehensive and coordinated physical therapy services to patients in their homes and communities.


You will:

  • Conduct comprehensive assessments of patients' physical abilities, including range of motion, strength, and coordination.
  • Develop and implement individualized treatment plans that are tailored to each patient's needs and goals.
  • Provide patient education and support to promote self-management and prevent complications.
  • Coordinate care with other healthcare providers, including physicians, nurses, and social workers, to ensure continuity of care.
  • Document all patient care activities and communicate patient status and progress to the healthcare team.
  • Participate in quality improvement activities and initiatives to improve patient outcomes and satisfaction.
  • Maintain professional knowledge and skills by attending continuing education programs and staying up-to-date with the latest developments in the field.
  • Adhere to all applicable laws, regulations, and ethical standards of physical therapy practice.

Qualifications:

  • Current and unrestricted PT license in the state of Florida.
  • Minimum of 2 years of clinical experience in a healthcare setting, preferably in home health or community health.
  • Strong assessment, critical thinking, and problem-solving skills.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare providers.
  • Ability to work independently and manage multiple priorities in a fast-paced environment.
  • Proficient in using electronic health records and other healthcare technologies.
