Position Overview:
Priority Group Services is actively seeking a skilled and compassionate Physical Therapist to join our esteemed team in New Jersey. The Physical Therapist will play a pivotal role in delivering exceptional care, encompassing both rehabilitation and habilitation services. This role requires adherence to professional standards and regulations while contributing to the overall well-being of our patients.
Responsibilities:
Patient Assessment and Treatment:
- Conduct comprehensive assessments to evaluate patients' conditions and needs, focusing on both rehabilitation and habilitation goals.
- Develop personalized treatment plans that aim to enhance mobility, alleviate pain, and improve overall functionality, addressing the unique needs of patients in both rehabilitation and habilitation settings.
- Implement evidence-based physical therapy techniques and exercises tailored to meet individualized goals.
Progress Monitoring and Reporting:
- Monitor patients' progress closely, reassessing as necessary, and modifying treatment plans to ensure optimal outcomes in both rehabilitation and habilitation scenarios.
- Maintain detailed and accurate patient records and progress reports, documenting achievements and adjustments for each stage of the therapeutic process.
- Patient Education and Support:
Educate patients and their families on therapeutic exercises and strategies, emphasizing skills essential for managing conditions or injuries in both rehabilitation and habilitation contexts.
- Provide unwavering emotional support and encouragement throughout the rehabilitation and habilitation processes, fostering a positive and empowering therapeutic relationship.
Collaboration and Communication:
- Collaborate seamlessly with multidisciplinary teams, including physicians, nurses, and other therapists, to ensure a coordinated approach to patient care in both rehabilitation and habilitation settings.
- Communicate effectively with healthcare professionals and caregivers, providing comprehensive updates on patient progress, needs, and achievements in both rehabilitation and habilitation realms.
Requirements:
- Possession of a New Jersey Licensure as a Physical Therapist is mandatory.
Compliance with all federal, state, and local regulations, including those set by the New Jersey Department of Health, Office of Inspector General, Medicaid/Medicare, Corporate Compliance, PEOSHA, and Priority Group Services Policies and Procedures.
- Strong interpersonal skills and the ability to build rapport with patients.
- Excellent communication and documentation skills.
- Proficiency in utilizing relevant physical therapy equipment and techniques, with a focus on both rehabilitation and habilitation methodologies.
Preferred Qualifications:
- Experience in a clinical setting
- Familiarity with electronic medical records systems.
- Specialized certifications or training related to specific areas of physical therapy.
