Job Description:

Responsibilities:

• Perform plan of care interventions, including medication administration, specimen collection, and clinical treatments.
• Document patient care and communicate patient conditions to the care team, physician, patient, and family.
• Assist patients with activities of daily living, including personal hygiene, elimination, nutrition, and ambulation.
• Collaborate across disciplines to coordinate patient care, including transfer, discharge, referral, and spiritual/psychosocial support needs.
• Evaluate the learning needs of patients and provide education appropriate to age, culture, condition, and circumstances.
• Advocate for the physical and emotional well-being of the patient.
