Eminence Home Care CT is seeking a Full-Time Registered Nurse (RN) to join our team. Responsibilities
- Conduct in-home assessments and create personalized care plans for clients
- Administer medications and monitor vital signs
- Provide wound care and other skilled nursing services as needed
- Educate clients and their families about disease management and prevention
- Communicate regularly with physicians, healthcare providers, and family members
- Work collaboratively with a team of healthcare professionals to deliver high-quality care
- Maintain accurate and thorough documentation of all services provided
Requirements
- Current RN license in the state of Connecticut
- Minimum of 2 years of experience in home care or a related field
- Strong assessment and critical thinking skills
- Ability to work independently and as part of a team
- Excellent communication and interpersonal skills
- Reliable transportation and valid driver's license (preferred)
- Ability to pass a background check and drug screen.
