The Registered Nurse (RN) Case Manager role at Enhabit involves performing visits, coordinating client care, and serving as the main contact for all involved in providing care. This includes consulting with physicians and providing details about patient care. Key responsibilities also include accurate OASIS collection, maintaining current medication profiles, timely reporting of lab values to physicians, and attending case conferences and manager meetings.
To qualify as a Registered Nurse (RN) at Enhabit, you must:
- Currently hold a valid Registered Nurse (RN) license in the state of employment
- Preferably have at least one year of clinical experience
- Demonstrate knowledge and skill in current nursing practice
- Hold a valid state drivers license and automobile liability insurance
