As a Clinical Team Leader at Enhabit Home Health & Hospice, you will have the opportunity to coordinate home health services and ensure they meet acceptable standards of practice. You will review and approve patient information, assist with case conferences, review and approve orders, and oversee patient care. Your role will also involve managing the office and staff related to patient care and providing education and training to clinicians to meet regulatory standards. You will play a vital role in facilitating relationships between physicians, referral sources, patients, caregivers, and employees.

To be eligible for this position, you must be a graduate of an accredited school of professional nursing and licensed in the state of employment. You should have at least two years of nursing experience, including one year of experience in home health. A good understanding of the laws and regulatory guidelines governing the operation of a Home Health agency is essential. Proficiency with technology, including general office equipment and electronic medical record platforms, is preferred. You must also possess a valid state driver's license and automobile liability insurance and be comfortable driving in all types of weather conditions.
