The Long-Term Services and Supports (LTSS) area of Human Services and Public health department is looking for two Case Management Assistants (CMA) to join our team. These positions will work on an interdisciplinary team of Public Health Nurses and Social Workers to provide follow-up to individuals assessed for the LTSS area. The CMA will focus on the post assessment work needed to access programs and services after the assessor completes the assessments and plan. We take a person-centered approach to our work. This team is a part of administers Medicaid waiver and state programs serving frail seniors, and individuals with brain injury, mental health, complex medical, developmental, and physical disabilities. Effective December 31, 2023, there will be a general salary adjustment of 2.5% to the minimum and maximum of the range. Location and hours: These positions are remote. Remote workers may be required to come on-site 0 to 12 days a year for work meetings, trainings, or for any other reasons that their supervisor may deem necessary (may include additional days for onboarding and new employee training).Supervisors will provide as much advance notice as practicable. Work hours will be Monday through Friday from 8 a.m. to 4:30 p.m. Based on business needs of this position, hires must live in or relocate to Minnesota.About these position types: These are full-time, benefit earning positions. You must be a current regular or probationary employee of Hennepin County to apply for these positions. Limited duration employees are not eligible.These positions are internally classified as Case Management Assistants.Click here to view the job classification (link removed) these positions, you will: Follow up with clients to gather information and provide assistance to complete the county and state documents needed to assure payment to deliver services and supports.Review files and obtain client information from multiple county and state databases such as Social Services Information System (SSIS), electronic case files (ECF), and Medicaid Management Information System (MMIS) as well as managing data in Microsoft Excel, and other Microsoft applications, or other reporting and managing systems.Manage and maintain client related records or other information records, collect and track demographic data and ongoing service and payment data.Monitor ongoing cases with direction from Public Health Nurse (PHN) and Social Worker (SW) by obtaining periodic case update information and entering information on case files, follow up with clients who fail to return completed eligibility documentation, and complete follow-up documentation after PHN or SW determines a need for closure for participants who do not meet ongoing eligibility criteria.Collaborate with external collaterals and internal business partners to acquire and update documents needed to assure payment of services, assist with appeals and smooth transitions.Maintain and organize data for reports for assisting with managing workflows.Stay informed/current on state statute and county procedures that impact the work of their support to service delivery and focus.Participate in teams effort to ensure processes, protocols, and program are in compliance and meeting outcomes. Need to have: One of the following: Two years or more of responsible, paid social service experience providing direct client service.A two-year human services certificate.Two years or more of college coursework in the behavioral sciences.Nice to have: Experience: Using databases and computer programs such as SSIS, ECF, and MMIS to perform data entry where accuracy is essential.Working with multiple providers, staff, and collaborative partners to improve the care of diverse and underserved people with a variety of cultural and language barriers.Using Microsoft Office Suite (Excel, Outlook, PowerPoint, Teams, and Word).Ability to: Work effectively and communicate with individuals from a variety of cultural and lifestyle backgrounds in a respectful and non-judgmental manner.Make sound, independent judgment.
