Senior Case Manager

EAC Network, a not-for-profit social service agency that empowers, assists, and cares for over 65,000 people in need through 100 programs across Long Island and NYC, seeks a Senior Case Manager to work full time for its Nassau Treatment Alternatives for Safer Communities (TASC) Program.

The Senior Case Manager position is a non-exempt position paying $25-27 per hour. The hours of operation are Monday-Friday 9:00AM-5:00PM. The office is located in Hempstead, NY. The role reports to Program Director.

Primary Purpose of Job:

The Senior Case Manager provides services and identification, screening, treatment planning, case management, and court reporting of eligible clients for the Treatment Alternatives for Safer Communities (TASC) Programs and acts as resource to staff.

Principal Duties & Responsibilities:

  1. Act as liaison to the courts and correctional facilities. Provide case-findings, assessments, and screening services for cases within the judicial system for clients involved with alcohol or drug abuse and/or presents evidence of mental illness in order to ascertain appropriate treatment as an alternative to incarceration.
  2. Follows and implements a comprehensive screening assessment, to effectuate referrals to a mental health, drug abuse, or alcoholism facility and accompanies and/or arranges transportation for clients to the appropriate facility when necessary.
  3. Maintain case documentation available for review by supervision at any time, i.e. client records, including progress notes and all other relevant information to assist clients and to comply with program requirements.
  4. Assist clients in applying for and receiving public and private benefits.
  5. Maintain a caseload of offenders and monitor their participation in the treatment program through case management appointments, reviewing progress reports and conferencing cases with treatment personnel.
  6. Make periodic visits to treatment facilities and maintain accurate log of visits.
  7. Transport client to agency and other facilities.
  8. Prepare and present written and oral status reports to the criminal justice system in a timely manner regarding client's progress.
  9. Investigate and recommend supportive services for clients.
  10. Conducts educational and/or supportive groups as needed including Evidence Based Practices (EBPs).
  11. Conduct drug testing on clients of the Program as needed and timely visits to treatment and correctional facilities
  12. Act as a resource to staff members for guidance in treatment planning and treatment needs.
  13. Train and supervise interns and volunteer staff.
  14. Assists supervisor with administrative tasks related to program management.
  15. Participate in on going trainings and possible conferences determined by supervision.

Knowledge, Skills, & Abilities Required:

  1. Associates degree in a helping profession and at least 2 years of prior experience working in intervention programs with criminal justice involved participants. Bachelors degree preferred in criminal justice, social work, counseling, forensic psychology, psychology, sociology, or criminology and one year or more of experience in substance abuse, criminal justice or working with the substance abusing and/or mentally ill population.
  2. Good communication skills, both oral and written. Ability to work cooperatively with treatment and criminal justice systems.
  3. Must be proficient in computer applications or other automated systems such as spreadsheets, Microsoft Office applications, calendar, and email and/or database software necessary to perform work assignments.
  4. Must have NYS State Drivers license and access to vehicle.
