Job Purpose: Provide case management for about 15 youth in custody or out in community. Youth ages 14-24, who identify with being disconnected, in-risk, system involved, on probation and in crisis. ReSET will serve as an alternative to incarceration that achieves outcomes such as: lowering recidivism rates, those on probation will successfully complete and youth previously engaged in violence will not re-engage. Case management services include but not limited to-court advocacy, reentry planning, linkage to resources, life skills training and other workforce and education services.

Requirements

Minimum Qualifications:

  • Must have 4+ years case management experience
  • Must have 4+ years experience working with court involved youth, and juvenile and adult probation departments and corresponding court systems
  • Must have experience serving undocumented youth, including familiarity with current immigration law, and disconnected high school and transitional aged youth
  • Must have valid CA drivers license and vehicle
  • Must be familiar with the framework or practice of Youth Development and Restorative Justice; knowledgeable with process of sealing and/or expungement of criminal records
  • Must have knowledge of the issues plaguing bay area children and youth to better supervise and support the diverse and often high risk population CHALK serves

Responsibilities:

  • Conduct various assessments of each youth; assess them in the areas of risk, academics, workplace skills, and/or behavior; identify barriers, strengths and individual characteristics
  • Create stabilization plans to meet and address barriers using internally developed case management tools
  • Meet with youth in person weekly, depending on need and risk assessment, to formally monitor progress of stabilization plans; check in via phone call, text and email on a daily to weekly basis for informal monitoring
  • Work with youth in setting educational, professional and behavioral short and long term goals; enroll youth in evidence-based Thinking for Change (T4C) cognitive behavior modification training
  • Responsible for maintaining case notes on each client; accurately and regularly input data into spreadsheets and funder databases to maintain participant information in accordance with BACRs standards and any program-specific funder(s) requirements
  • Make referrals to other services and programs as appropriate; attend and/or bring client to such activities when necessary.
  • Engage with youth in cultural healing activities
  • Attend school meetings, court hearings and other appointments influencing the clients; act as advocate when appropriate
  • Responsible for writing character and recommendation letters for court on behalf of participants
  • Support clients families as appropriate; assist with strategizing with families and lawyers when clients are facing charges; provide translation at court, etc.
  • Participate in coordinating collaborative case conferencing sessions; facilitate phone and in person meetings between stakeholders that are each serving/supporting participants; collect status updates and additional data on participants from partners
  • Work closely with academic and career coaches through career pathways department ; Ensure data/documentation collection and reporting is complete for each participant on your caseload
  • Represent BACR at internal/external networking and partnership meetings/events
