We are currently looking for experienced Registered Nurses (RN) to join our team! This is a PER DIEM position and paid on a per visit basis for Starts of Care (SOC), Re-Visits, Re-Certifications and Discharges. There will be the opportunity to grow into a Part-Time or Full-Time position in the near future!

• Start of Care (SOC): $110


• All Other Visits: $60.00


• Weekly Pay with Direct Deposit

RESPONSIBILITIES:

• Conduct an initial and ongoing comprehensive assessment of the patient's needs, including Outcome and Assessment Information Set (OASIS) assessments at appropriate time points;


• Obtain a medical history from the patient;


• Evaluate the patient and home situation to determine what health teaching will be required, if additional services are needed and the frequency of this service;


• Develop and implement the nursing care plan;


• Document services and education provided to the patient, using an Electronic Medical Record (EMR) application; No Place Like Home Health uses Alora Plus; MUST be able to adhere to strict deadlines for Start of Care (SOC), revisits, re-certifications and discharges;


• Coordinate total patient care


• Participate in Case Conferences, In-Services and mandatory Staff Meetings as needed.

MINIMUM REQUIREMENTS:

• Must have a valid, unencumbered Registered Nursing License from the State of Michigan;


• Must have a minimum of two (2) years of Skilled Home Care / OASIS Visit nursing experience;


• Must have a valid Drivers License, your own reliable transportation and proof of current Auto Insurance;


• Must be willing to travel within a designated radius (from your home address); this radius will be agreed upon during the interview process and upon hire.
