Job Title: Smoking Cessation Care Coordinator/Coach
Location: Office (Determined by Management)
Position Reports to: Director of Operations/Clinical Supervisor
Job Purpose Summary: Provide telemedicine visits for smokers who are Louisiana residents interested in quitting smoking
Key Responsibilities and Expectations:
Some of your duties and/or expectations as a Smoking Cessation Coordinator include, but not limited to the list below:
• Communication: Communicate clearly and in a friendly manner via phone and in person
o Complete the application and consent
o Coordinate appointment dates and times with patients and providers
o Conduct telemedicine visits with patients and providers
o Ensure patient prescriptions are handled and received by patient
o Handle patient benefits thru Smoking Cessation Trust
• Data recording: Complete smoking cessation trust patient applications, patient consents, requests for additional patient benefits, enter demographics, problems, medications, medication allergies on exam forms and in Practice Fusion (Electronic Medical Records), edit, update, and maintain smoking cessation patient call list, edit, update and maintain smoking cessation master billing spreadsheet, upload all documents to Practice Fusion, complete all electronic forms within Cognito, submit Outlook calendar invites for all scheduled appointments and needed patient follow-ups, add scheduled patient appointments to provider calendars within Practice Fusion, submit all patient surveys, etc. Proficiency in utilizing Excel, Outlook (Calendar and Email), Power Point, Publisher, Word, and any other program needed to meet the Company’s needs is required. If further education is needed to become proficient, employee will be required to cover the cost for education and will need to complete education during employee’s non-working hours/time
• Time Management: Ensure all scheduled appointments are conducted as scheduled, ensure providers are available for scheduled appointments to ensure patient’s time is respected, ensure all patient appointments are scheduled as ordered by the providers and/or protocol set, submit requests for additional benefits with the smoking cessation trust by the deadline set, update all documents, forms, spreadsheets, calendars, etc. daily
• Patient satisfaction: Strives for patient satisfaction and retention in the Chronic Care Management program by:
o Completing patient surveys
o Building personal relationship with patients
o Ensuring patient’s smoking cessation needs are met
o Providing smoking cessation educational material to the patients
Job Type: Full-time
Salary: $9.00 /hour
