Full-Time Remote, Telephonic opportunity
The Telephonic Social Worker assesses and evaluates members' needs and requirements to achieve and/or maintain an optimal wellness state. Using their knowledge of community and social services resources, they guide members and their families toward and facilitate interaction with resources appropriate for the care and wellbeing of members. The professional in this role will rely on their experience with Social Determinants of Health (SDOH) to handle assignments which are varied and frequently require interpretation and independent determination of the appropriate courses of action.
Responsibilities
The Telephonic Social Worker employs a variety of strategies, approaches and techniques to manage a member's physical, environmental and psycho-social health issues. Responsibilities include the following:
Identify and resolve barriers that hinder effective care.
Ensures patient is progressing towards desired outcomes by continuously monitoring patient care through assessments and/or evaluations.
Will create member care plans.
Assess members' social needs related to housing, food, transportation, safety, etc. and as-needed refer them to community-based resources.
Understands the department, segment, and organizational strategy and operating objectives, including their linkages to related areas.
Makes decisions regarding own work methods, occasionally in ambiguous situations, and requires minimal direction and receives guidance where needed.
Follows established guidelines/procedures.

Required Qualifications
Master's degree in Social Work(link removed)>
Current valid social work license, e.g. MSW, LCSW, LMSW
Minimum 2 years of experience in identifying and accessing resources to meet basic needs and in securing community-based assistance.
Must possess effective telephonic and virtual communication skills.
Proficiency with Microsoft Office Programs, e.g. Word, Excel, and MS Teams
Schedule: Monday-Friday, 8-hour shifts between 9AM and 8PM EST. Based on business needs, occasional weekends may be required.
Ability to obtain additional state licensure
Work-At-Home Requirements
Must have the ability to provide hard-wired, high-speed DSL or cable modem for a home office.
Satellite and Wireless Internet service is NOT allowed for this role.
Associates or contractors who live and work from home in the state of California will be provided payment for their internet expense.

Preferred Qualifications
3-5 years of experience working as a licensed Social Worker
Experience with behavioral change, health promotion, coaching and wellness
Experience working with adults, and aging population preferred
Knowledge of community health and social service agencies and additional community resources
Bilingual English/Spanish. Must be able to speak, read and write in both languages without limitations or assistance.

Pay Range
The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay decisions will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
$57,700 - $79,500 per year
This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
