Impact Life is looking for Transitional Housing Specialist to join our team in Delaware. The Transitional Housing Specialist will facilitate and evaluate admission and discharge arrangements of all incoming and outgoing residents. In addition, the Transitional Housing Specialist will work with family members through the admission and discharge process.


Please note this position will require the ability to be on-call during the evenings and weekends.


Responsibilities

  • Respond to all incoming admissions in DTRAN, OpenBeds, and other means of referrals.
  • Conduct an assessment to gauge appropriateness and confirm level of care for the resident.
  • Coordinate with Medical Director and Housing Team to place the resident.
  • Coordinate admissions into our programs.
  • Works with residents who have planned discharges to ensure a smooth transition to their next level of care.
  • Connects all discharges residents to our alumni program.
  • Provides support to alumni.
  • Provides appropriate orientation to program aspects post admission, including documentation and data input into computer system.
  • Assures prompt responses to resident needs, reviews and maintains all documents in the approved EMR.
  • Provides initial contact with referral sources and potential residents.
  • Conducts telephone and/or in-person screening and assessments.
  • Coordinates the referral and intake process to assist with determining the appropriate level of care and
  • Maintains communication with referral sources, families, patients and staff.
  • Collects and analyzes data related to departmental performance indicators.
  • Assists counselors, case managers, and other program staff in crisis intervention as needed.
  • Work with residents who are attempting to leave the program abruptly and coordinates care for them when necessary.

Requirements

  • Bachelors degree in Human Services, Behavioral Health, Psychology or related field required.
  • Minimum of 2 years experience working in Admissions for substance use disorder program provider required.
  • Knowledge of HIPAA policies and procedures
  • Proficient with Microsoft Office.
  • Strong communication and negotiation skills.
  • Demonstrates concern for meeting internal and external stakeholder needs in a manner that provides satisfaction for them within the resources that can be made available.
  • Communicates information to individuals or groups clearly and concisely.
  • Able to convey information orally or in writing to individuals or groups to ensure that they understand the information and the message.
  • Ability to listen and respond appropriately to others.
  • Ability to establish effective working relationships that foster organizational success.
  • Maintains high standards despite pressing deadlines.
  • Regularly produces accurate, thorough, professional work.
  • Understands duties and responsibilities, has necessary job knowledge, has necessary technical skills, understands company mission/values, keeps job knowledge current, and is in command of critical issues.

  • Requires travel throughout the State of Delaware
