VAD Coordinator
Shift Details: Monday – Friday, 8 am-4:30 pm.
Has Relocation in a case by case basis
Position Summary:
- Participates in the clinical management of pre and post implant patients in collaboration with the APPs and physicians.
- Coordinates the clinical evaluation of potential VAD candidates.
- Coordinates discharge plans and verifies patient knowledge base prior to discharge.
- Provides follow-up consultation for discharged patients and sees patients at clinic visits.
- Educates nursing staff and patient/caregiver on patient care, maintaining VAD equipment, and safety measures.
- Provides education and support to community health care professionals who may encounter VAD patients.
- Serves as a clinical expert and leadership consultant to nursing, members of Healthcare and to the larger community.
- Ensures compliance with state, federal and organizational guidelines and other regulatory agencies.
- Provides education and support to community health care professionals who may encounter VAD patients.
- Acts as a consultant to referring community health care specialists and primary care providers.
- Assures that all VAD patients have an identified PCP and that these MDs receive correspondence and educational material on their VAD patients.
- Develops resource materials for referring physicians.
- Develops strategies to improve communication between the VAD program and outside providers.
- Manages VAD patient population to promote efficient, cost effective, quality care to both the patient and Healthcare.
- Develops policies and procedures for the delivery of care to the VAD population.
- Ensures compliance with state, federal, organizational and other related regulatory agencies.
- Works with the financial coordinator to provide information and assess insurance coverage for devices and post implant supplies/resources prior to device implant.
- ·Provides input on financial strategies and operational planning issues related to the VAD population.
- Educates patients, families and nursing staff regarding VAD evaluation process, clinical management of patients, discharge planning and ongoing education to promote health.
- Provides education, including risks and benefits, to potential VAD candidates and post implant patients throughout inpatient and outpatient phases.
- Assesses and analyzes clinical findings to result in optimum goal achievement for patients in a cost effective manner.
- Collaborates with clinical pathway team and or appropriate leadership and implements procedures to improve efficiency and effectiveness.
- Evaluates competency of clinical staff (inside and outside of the hospital), patients, family members, physicians, and community health resources related to the care of patients with VADs.
- Responsible for the development of VAD patient and caregiver educational materials.
- Coordinates discharge plan to transition patient to the outpatient setting.
- Provides after-hours emergency call to patients and caregivers, to include nights, weekends and holidays, with oversight by the Advanced Practice Providers and physicians.
- Attends continuing education activities to maintain knowledge of current trends and practices.
- Documents appropriate information in patient's medical record.
- Maintains a patient management system and collects and documents required regulatory and program specific data.
- Acts as a consultant within facility and the VAD community.
- Identifies competency/learning needs and collaborated with department director, clinical coordinators and staff in designing orientation and on-going educational activities related to care of VAD patients.
- Develops education materials and provides classes for nursing staff and VAD team members.
- Performs other related duties as required.
Qualifications:
- BLS certification required and ACLS may be required in designated departments.
- At least 24 months of recent nursing experience within the past five years in acute care hospital setting.
- Acute care experience (critical care/transplantation/VAD experience) preferred.
- Ideal candidate will have three – five years of recent clinical experience as a Registered Nurse including cardiology or cardiac surgery with an emphasis on CCU/CVICU if possible.
- Currently licensed as a Registered Nurse in the state of Georgia.
