JOB SUMMARY

The Veterans Case Manager will work to ensure that any veteran living in homelessness in Salt Lake County is engaged and has access to housing resources. This position will ensure that men, women and families who have served our country and are living at The Road Home, on the streets or in a partnering shelter are supported as they overcome homelessness. The goal of the SSVF program is to provide low-income veterans experiencing homelessness or at-risk of experiencing homelessness with a range of supportive services designed to promote housing stability. The Veterans Case Manager will work to identify common barriers faced by veterans and establish strategies and procedures for addressing needs on an agency-wide scale.

JOB DUTIES

1. Provide direct case management, including crisis intervention, assessments, goal setting, problem solving, agency and community resources utilization, and network building.

2. Conduct and complete initial eligibility screenings/needs assessments and recertifications, perform paper and digital intakes in ClientTrack, create case files with required documentation for all clients

3. Collaborate with program supervisor in ensuring program fidelity, documentation accuracy, and intra-agency collaborations 4. Responsible for helping the veteran create housing stability plans to include the provision of housing placement services, temporary financial assistance, supportive services, and community referrals as needed

5. Conduct follow up calls/visits with SSVF approved veterans to ensure they are supported in reaching their housing stability goals

6. Engage and build rapport with the target population, other community outreach programs, local social services and public benefit agencies, shelters, faith-based and community-based organizations, and local VA facilities

7. Provide resources and link veterans to community resources to remove or minimize barriers to retaining housing

8. Attend weekly collaboration meetings, including the Veterans Community Triage group and with housing navigation teams

9. Engage with all clients during daily open office hours to connect them with resources

10. Set and maintain healthy boundaries with clients

11. De-escalate, empower and advocate for clients in landlord mediation.

12. Keep records, case management notes, and statistical data as required and participate in research and/or special projects as needed.

13. The ability to engage and encourage housing conversations with clients

14. Assess barriers to housing including helping guests get access to vital documents, identifying lease requirements, and assist with obtaining or retaining income options

15. Moving guests and their belongings to their new homes.

16. Use agency vehicles to provide transportation for guests.

Requirements

a. S.A.V.E. Suicide Prevention

b. Fraud, Waste, and Abuse

c. SSVF RRH 101

Education about different roles and responsibilities within local SSVF agency

Trainings should include discussion of culture, e.g., race/ethnicity and Veteran culture
