Pay Rate: $24.18

RADIAS Health is seeking a part-time Mental Health Practitioner to join the Community Care Service Team (CCST) - flexible schedule available! This position specializes in supporting individuals who have been identified as high utilizers of emergency room/hospital services and/or have been identified as someone with high unmet needs and would benefit from additional support. The position is hybrid with the opportunity to work from home when not meeting with individuals in the community. The team uses a harm reduction and a clinic without walls approach.

Some duties include: developing, writing, implementing, evaluating and revising of treatment goals and plans in collaboration with the client and the treatment team, assisting individuals in establishing additional community supports, and communicating with natural supports. The Practitioner will coordinate services with other providers including, but not limited to, local community hospitals, Mental Health Case Management, outpatient behavioral health providers, lawyers, guardians, care coordinators, and probation officers. Practitioners on this team have the opportunity to observe diagnostic assessments and Rule 25/comprehensive assessments

**LPC/LPCC/LADC clinical supervision hours can be provided by the manager**

Schedule: 20 hours per week, Monday - Friday (Friday half 8 am - 12 pm), schedule is flexible

Requirements

  • Bachelor's degree in behavioral sciences or related field (if no degree, 30 semester credit hours in behavioral health sciences) and 2,000 hours supervised work experience with individuals diagnosed with serious mental illness and/or substance use disorders - OR-
  • Masters or other graduate degree in behavioral health sciences or related field -OR-
  • If no degree, must have 4,000 hours supervised work experience with individuals diagnosed with serious mental illness and/or substance use disorders
