JOB TITLE: RN Case Manager UAS Assessment Nurse

Duration: 3 months+

Location: MUST be located in the New York metro area (will be traveling through all 5 boroughs)

Pay: up to 57/hour

• MUST be an RN
• MUST be UAS certified with minimum of 1 year experience

Job Description:

• Looking for UAS certified RNs in New York Metro area to conduct and administer the New York State Uniform Assessment Tool (UAS) Comprehensive Health Assessment for Long term Care members.
• Assessments wil be done virtually or in-person, based on the member's needs. Currently the majority of assessments are being done VIRTUALLY.
• MUST be located in the New York metro area
• MUST be an RN
• MUST be bilingual (English/Spanish, English/Bengali, or English/Chinese)
• MUST be UAS certified with minimum of 1 year experience
• Needs to be UAS user role 40 or 45
• Nurses will be servicing the borough in which they are located in! They will not be required to travel throughout the whole city, if and when travel is required
• The hours per week should run at 40 for the contractors to include the supplemental paperwork and forms this will usually factor out to 3 assessment a day approx. at max 2 hours an assessment to include supplemental forms which are all also electronic forms.
