The Complex Care Nurse ensures a client and his/her caregiver are central to the role in coordinating and managing care; collaborates closely with the primary care provider and assures comprehensiveness of care across disciplines; engages external partners in coordinating care as necessary; and, enhances access to quality care regardless of payor source while reducing healthcare costs.The Complex Care Nurse is a nurse specialist that works with medical, behavioral, and socially complicated patients and functions at the top of his/her scope. This role draws on a depth of understanding in the areas of pathophysiology, pharmacology, psychiatric nursing, family systems, health systems, and data analytics. The Complex Care Nurse at MFHC in many cases will carry the broadest or most comprehensive view of an individual complex care patient and subsequently be most informed in guiding interdisciplinary planning for care of that patient.JOB QUALIFICATIONSEducation or Formal Training & Experience:Registered nurse; 2 years of experience required Home health or community health experience desiredCurrent Colorado license.BSN required (MSN encouraged) Current B.L.S. certification. Certification in case management (eg CCM, ANCC RN-BC, ACM) to be obtained within 3 to 5 years of hire date.Certification in Motivational Interviewing (preferred).
