At Citadel at Home, we provide quality in-home healthcare services to our clients. We are seeking a Home Care Coordinator Supervisor to join our team! In this role, you will be overseeing a team of home care coordinators and will be responsible for ensuring the smooth delivery of care services to our clients. You will also collaborate with our healthcare professionals to guarantee our clients are receiving the care they need. The ideal candidate should be passionate about providing high-quality care and have excellent communication and organizational skills.
Responsibilities
- Oversee a team of home care coordinators
- Monitor referrals and ensure clients receive services in a timely manner
- Collaborate with healthcare professionals to ensure client needs are being met
- Create care plans that address clients' needs and preferences
- Maintain clear and accurate records in our information system
- Handle client complaints and ensure prompt resolution of issues
- Train and provide guidance to new home care coordinators
Requirements
- At least 3 years of experience in home healthcare coordination
- Excellent communication and organizational skills
- Strong leadership and problem-solving skills
- Comprehensive knowledge of healthcare laws and regulations
- Ability to work independently with minimal supervision
- Proficient in Microsoft Office and electronic health records (EHRs)
