Touchstone Health is a full-service home care and hospice company providing a broad range of services to its customers. Our leadership team is currently seeking an organized and detailed OASIS, Quality & Compliance Nurse Educator (RN) (LVN) with a background in OASIS coding and quality assurance in home health. This individual should be able to provide education to our new nursing staff and ensure that our agency is adhering to both state and federal guidelines for quality control. Duties & Responsibilities:

• Perform review of Oasis assessments and Plan of Care completeness and accuracy with coding.
• Able to use SHP as a tool to review Oasis answers and coding rules.
• Develop a QAPI Plan in accordance with current rules, regulations and guidelines that govern Home Health and Hospice.
• Assist in education of clinical staff as it relates to Oasis items and accuracy.
• Conducts needs assessments using a variety of Strategic tools
• Assists learners in the assessments of needs and identification of outcomes
• Incorporates principles of adult learning in the planning, implementation, and evaluation of educational programs
• Coordinates orientation of new staff with Human Resources department
• Develops/Coordinates core education to address patient population specific care
• Develops/Coordinates education for the development of Home Health/Hospice leaders
• Plans, Prepares, and maintains materials for use in educational programs for accuracy and relevance to target audience
• Identifies and evaluates instructional materials in all types of media and formats
• Adjusts content and teaching strategies when appropriate for learner needs
• Provides educational opportunities that benefit nurses at all stages of their career development
• Delivers competence-based programs for staff with a focus upon improvement of skills and retention of knowledge
• Prepares short and long-term planning and programs to address identified needs
• Teaches content as needs and/or assigned within clinical expertise to facilitate orientation, staff development and/or safe practice

• Ensure correct documentation to maintain a 4.0 STAR rating and higher.
• Audits clinical documentation to ensure quality regulations and measures are being met

Requirements

Qualifications & Requirements:

• Current and unencumbered license to practice in the state of Texas
• Minimum of three (3) years of home health nursing experience
• OASIS Coding Certification preferred
• Current CPR certification
• Reliable transportation, valid driver's license with no DUI's, revocations, or suspensions in the last 5 years
• Excellent written and verbal communication skills
• MS Office skills (Word, Excel, PowerPoint
