Citadel at Home, a leading provider of home-based medical care, is looking for a Home Care Nurse Scheduler to join our team. This is an excellent opportunity to work in the Hospital & Health Care industry and to be part of a company that provides high-quality, patient-centered care. The Home Care Nurse Scheduler will be responsible for ensuring that our patients receive timely and effective care by efficiently scheduling visits for our nurses. The successful candidate will be detail-oriented, highly organized, and possess excellent communication and time-management skills.

Responsibilities

• Manage the scheduling of home care visits for nurses, ensuring that visits are scheduled in a timely and effective manner.
• Coordinate with nurses to determine visit frequency and timing based on patient needs and available resources.
• Ensure that the patient's care plan is followed, and review and revise the schedule as needed.
• Manage patient and caregiver communication to ensure visits are scheduled at convenient times.
• Work with the billing department to ensure accurate billing and reimbursement.
• Ensure compliance with regulatory requirements and company policies and procedures.
• Provide administrative support to the team as needed.

Requirements

• Previous experience in scheduling nursing visits or managing home care schedules.
• Excellent communication and interpersonal skills.
• Ability to work independently and manage multiple tasks and priorities.
• High level of attention to detail and accuracy.
• Strong organizational and time-management skills.
• Understanding of Medicare and Medicaid regulations.
• Familiarity with electronic medical records and scheduling software.
